We will offer you receipt once your treatment has been completed. The receipt will include all information required for your insurance claim. If you would like to have a printed copy, please inform us prior to your scheduled appointment time.

Most insurance policies allow us to direct bill, please check your insurance company policy if it is eligible to pay. For partial coverage, we can still direct billed. However, you will be required to pay the difference at the time of your treatment. Please bring your benefits card (including policy number, member ID) to your first appointment and allow us to set up account in the system.

Parking fees will be applied to service rates at any location requiring payment.
